Using the FAB Strategies Form to help Students with Developmental & Behavior Challenges

The FAB Strategies for Learning Readiness Form (FAB Strategies Form) can be applied in a variety of ways by therapists and teachers. FAB Strategies Form Description Blog Although the FAB Strategies Form is copyrighted therapists and teachers have my permission to use it with their students.

The FAB Strategies Form can be used as a quick checklist of evidence-based learning readiness strategies for students with developmental and learning challenges. The strategies are categorized into four sections: Environmental Adaptations, Sensory Modulation, Positive Behavior Support and Physical Self-regulation.

The most common use of the FAB Strategies Form is efficient development, revision and documentation of individualized strategies to achieve student’s goal-directed behavioral learning readiness progress compared to their baseline. Three lines on the bottom are provided for adding additional strategies. To save time therapists and teachers can list their commonly used strategies on these lines, include their name and contact information on top of the form, then make copies for developing student’s individualized programs.

It is helpful to include at least one strategy from each of the four categories. Chosen strategies are underlined while strategies to try during future sessions are marked in pencil. Equipment given is circled while equipment order is underlined. This is helpful for monthly consultation as well as weekly documentation. One form can be shared by all team members to literally keep everyone on the same page. FABWorkingSample

Still another efficient use for the FAB Strategies Form is revising it using my Word template to develop home, assistant teacher, and teacher/therapist as well as school record documentation forms. As shown in this sample form assessment information can also be included FABSTRATFORMGirlSevAutSample copy

Home programs begin with only one or two strategies listed which will most improve behavior. It is also useful to get the signature of the parent/guardian on the bottom of the form signifying that they understand and approve of the strategies and equipment given. Similarly, a program can be developed for para-professionals to assure the program is done more frequently. The FAB Form can also be typed for inclusion in the school records. Finally, sending it with your contact information when the student transitions to another grade, school, or program enables them to benefit from the strategies you found most helpful.

Through slight adaptations the FAB Strategies Form can be quickly used: as a checklist of techniques that can be selected to improve behavior and learning, to document learning readiness baselines, develop goals and a learning readiness strategies program, document parental permission for use of program strategies and equipment, documentation, home programs for parents and staff, and a list of behavioral and learning readiness strategies when a student transitions to other teachers and therapists. I hope you will find the FAB Strategies Form helpful for saving time, documentation, individualized planning, team coordination, and sharing information. I’ve been using it in these ways for many years.
