FAB Strategies for Learning Readiness Form

The FAB Strategies for Learning Readiness form enables school therapists and teachers to develop an individualized program for their students that improves behavior and learning. It guides the school team in being consistent and literally “on the same page”. Despite its copyright all therapists and teachers have my permission to copy and use my new FAB Form to help their students by clicking the link below.

The top of the FAB form identifies the student, therapist and teacher. It specifies an objective functional goal as well as dated baseline data prior to addressing or modifying the goal. It then lists evidence-based strategies divided into four sections: Environmental Adaptations,

Sensory Modulation,

Positive Behavior Support,

and Physical Self-Regulation.

The bottom 3 lines of the strategies list has three activities lines where therapists and teachers can list all the strategies they use frequently that are not included on the form. Below this is a line including 4 websites that are excellent resources. From left to right they list my website, a site that identifies Tier 1-3 level interventions with assessment forms and treatment strategies for each level, then a reliable valid sensory processing followed by mindfulness resources website. The final line enables the student’s parents or guardians to sign that they understand and approve of the individualized strategies.
